Need Suggestions Creating Online Checklist

Hello Community,


My company has scheduled visitors so often that we have a internal word document checklist. 


The business flow is...


  1. Employee notifies that a visitor is coming and will need A, B, C, D ect...
  2. From their requests a checklist is printed with assignments to complete tasks for the visitors


I am trying to brainstorm or find existing PowerApp Checklist where

  1. Employee creates a new Visitor and checks off what they need A, B, C, D, ect...
  2. In the background I want to state
    • How many days prior task needs to be completed
    • Who owns each task and assign them a task in planner 
    • Preferably work out of powerapps 

I am noodling around using a SharePoint list to house all the Tasks, Owners and Time and then a second SharePoint to manage the requests but I'm hoping there is a smoother, streamlined, or exiting  way.
