Options to provision a customized SharePoint list form PowerApp

All, 

 

Any idea whether it is possible to provision customized SharePoint list forms with PowerApps?

We are in need to provision team sites on SharePoint, and on each of these sites the same document library is available. This document library needs to have a customized form containing some conditions as well as a button to start a MS Flow. 

 

I think the solution could work with just one MS Flow, independent of the amount of PowerApps that would initiate it. But is there any possibility to provision customized PowerApps SP list forms? 

 

Or any way to connect multiple libraries to the same PowerApp?

If you mean to save the SharePoint custom Form together with the site as a SharePoint template, then it is not available currenlty.

Sadly, we dont have this functionality right now. We cannot support migrating custom forms from one site to another or export/import via templates.

"

To have multiple Libraries connected into one PowerApps App is available.

With the Add data sources:
