Patch data back to sharepoint from HTMLText List

Hi,

 

I am currently in the process of making changes to a an app(Which i did not create previously) and need some help Patching information back to SharePoint. The way the app has been set up can be seen in the screenshots provided, essentially the "Add Item" button will add entries from the form into a list and displayed on the HTMLText part on the page. What im looking to do is then patch that information to a SharePoint list along with some text box data from previous pages, would anyone be able to give some help with this? I have provided the app screen, the submit button as it stands just now, the add item button and the columns within SharePoint. I should also add that there is an update button which does update the individual items in SharePoint as it brings back specific items based on the users email address at sign on, the issue is just patching new items/list of items back to SharePoint.

 

P.S i would have used a gallery personally but I'm trying to avoid extra work being done as the app is already created and i "Simply" have to implement it into our business environment.

Could you tell me:
1)whether you want to use this form to create a new record or update an existing record?

2)what is this form's mode?

3)what's the difference between clicking "add an item" button and "submit" button?

Please notice the syntax of SubmitForm function:

SubmitForm(Formname)

However, you use a variable in this function.

 

No matter you want to create a new record or update an existing record, the syntax is the same "SubmitForm(Formname)".

The key point is the form's mode.

If the form's mode is new, you will create a new record.

If the form's mode is edit  you will update an existing record.

 

What's more, please notice your required fields. These fields could not be updated with blank value.
