Power Apps form opening twice

My power apo suddenly started opening twice open clicking the SharePoint list item, see image below. It happened after implementing changes in below article. Any idea why?

Could you please check if have followed the steps 4 in the blog with no mistake? Please try below steps:

1\ In list settings -> Form settings, switch to use the default form, click OK to save, then go back and switch to use custom form again.

2\ wait a few minutes for the settings taking effect. Clear browser cache and see if the forms work correctly.

3\ If not, delete this custom form and customize the form from the beginning again.
