Power apps template

I am using a power apps template and I need to make it point to a data verse database and change the field names to the data verse table fields. If that does not work I can create a form using the datverse but need to copy and paste the template into my form and then update the form fields to fields in the data verse. Please provide clarification. I tried to copy and paste but it does not work very well.

Maybe you could try to replace your original field names with the field names you need.

is the above screenshot for a canvas app or a model driven app. I do not see a new screen option for model driven app.
