I have a Custom list with a Powerapp form used for Edit/Create/View.
Suddenly, the form has stopped laoding.
Users using Internet Explorer are reporting an issue, where when you click on New Item, the form starts loading but never stops as per attached screenshot.
We tried in Google Chrome and that works ok, the problem is I have already went a long way to make all users use IE for compatibility reasons.
I have attached errors from Developer tools/Console that pop up when the form is loading, not sure if thats any use to you.
Please help, i have a lot of users waiting to submit new reports.

@Jeff_Thorpe would you have a MS case number? That way if the issue is still continuing on Monday I can add another case to the pile and reference yours. Hopefully that will help move things along if there are more cases with the issue.

I had to move all my users to Chrome because of this. Absolute nightmare since we just about got them to use IE 11 to make another less important part of our Sharepoint work properly.
Please keep us posted on any updates.

Also having the same issue here, my 'Custom form' in SharePoint (online) is not loading in IE11 (.4) but is fine in Edge and Chrome.
I do get the same console error messages as @prae " SCRIPT5009: 'Promise' is undefined " if that is related?
This comment may be unrelated, but the form has not been loading in IE11 for a few weeks now, I originally thoiught it was a problem with *.powerapps.com not being in our trusted sites list, we manually added it and I'm pretty sure (memory failing) that the app worked after that. I spent the last few weeks vacationing and waiting for someone to add the site to group policy as trusted for all IE11 users in our enterprise. Now that's been done....the app won't load.
I would imagine I'd be getting many more hits online searches if this issue was affecting ALL custom forms in SP online but I can't figure out what is wrong with mine.
I'm happy to have a PowerApps tech run a remote session and dig around if that helps, I'd rather not tell my form users not to use IE!
Any more input would be greatly appreciated, I have teams breathing down my neck to start using this lovely looking form!
I was not going to hold my breath for Microsoft to fix this bug that they introduced, so I re-built the form as an app in PowerApps. This has the added benefit of being visible from the PowerApps mobile app. This did open up a new issue -- the mobile app has trouble getting the current username. I tried some workarounds but no success yet.
Secondly, for those still looking to open that form in IE 11, if you navigate into the SharePoint list itself, rather than navigate to a page with a list view, or via a link to that form, it does work in IE11. This may help some of you until a fix is delivered.

I just tried it as well and the power app worked for me again in IE 11 as well.
It looks like Microsoft just changed how the forms load. There is a circle loading sybmol now instead of the dots.
Same here, it looks like whatever was at issue has been resolved.
