Thanks for reporting your issue with loading the forms apps in IE. Unfortunately I'm not able to repro that issue with a new customized SPO Form App - it loads just fine.
Is this problem still occurring for you?
Are you able to send over some SessionId(s) showing the problem?

Thanks for providing the SessionId! I was able to find that session in our telemetry and it does appear truncated but with no obvious error. I've created a ticket for the appropriate team to investigate this further.
Something has changed and we now see a different "loading" screen and it no longer loads in ANY browser on ANY network (corporate, visitor, using mobile hotspot....) Please help!
