SQL Access permissions issue - Granted Everyone and still fails?

I have an app that I have expanded usage from 1-3 users to almost all my users. To enable this I first tried sharing it with an AD group. That didn't seem to work. Then I tried using Everyone and that still doesn't work. I finally tried adding the user directly and it still doesn't work. I am at a loss for why it isn't working?

Here is the message the users are getting:

This is the permissions I granted to app:

Finally here is the SQL connection permissions:

SQL Permissions GrantedSQL Permissions Granted

 

My only guess is maybe the allowed connections to Azure SQL is based upon site IP and the permssion request must be approved from there? I know I can access it from other IP but I have already granted myself permission. Any ideas?
