Sorry to lead you astray with the extra infomraiton.
1) I don't believe my Datasource has issues. When I refresh it from within the app, it doesn't throw any errors.
2) I have removed and re-added the data source. Just now as a matter of fact AND refreshed again with no issues.
3) I swear I have "inserted a new screen in your app and insert a new EditForm" without success in the past based on your suggestion, but this time it worked. I now do NOT have errors in my form.
4) Permissions shouldn't be an issue.
So, it appears that I am all set now, but don't understand why, which makes me nervous too. I'll take the win however. T

I can understand how these things can cause nervousness. For a better comfort, if you had time to play with it, there is something odd in that form that needs some attention. Either the fields are messed up and causing an issue, or there is something else. Again, if you had time, and now that you have a form that works, you could compare the two forms and their properties side-by-side and see where the offender is. That *might* bring some comfort in the process.
I may have spoken to soon. Here's where I'm at now. I deleted "SharePointForm1" and added in "Form1" then connected it to my SP datasource. The data entry fields still do not show errors, but when I run for testing, I get a blank screen. Is there a special way to insert a SharePoint form verses a Form?
After a little more investigation, it looks like if I publish the form it will show up when I click "New" in the list. It does not, however, show-up when I press the run button in PA.
Did you change the formulas in your SharePointIntegration?
If you change out the form in a customized form app, you need to alter those "default" settings.
There are "default" formulas in the OnCancel, OnEdit, OnNew, OnSave and OnView properties of the SharePointIntegration connection. Make sure they are all pointing to your new forms.
