Stop form from prefilling with SharePoint list item

Hello 

I have a very simple form.

Your Name (people picker), Your Area (choice), If other (text) and comments (multline text) and submit button.

In a SharePoint list, I click on New and it's lovely (see pic 1)

Using the App ID I have a SharePoint web part and put it on a page.

The form displays the name, area etc from the first item in the list. (see pic 2)

I go into PowerApps and it happens there as well. 

How do I stop PowerApps from prefilling with a previous item. I just want the field blank.

Could you show me how do you set this: "Using the App ID I have a SharePoint web part "?

I do not quite understand what is a SharePoint web part?

Do you mean that when you click "New", you want the SharePoint web part display blank value like pic1?

If the SharePoint web part is a form, please set its mode to New when you run the custom form.

set the SharePointIntegration's OnNew:

NewForm(SharePointForm1);NewForm( SharePoint web part form name)

Hi @xxxxmints ,

Make the default mode of the form FormMode.New.

When you save the form

SubmitForm(<FormName>);
ResetForm(<FormName>);
NewForm(<FormName>)

Is this what you are after?
