Submit from canvas app using patch function creating multiple items in SharePoint for a user

Hi Community team,

Hope you all are good.

Canvas app has some input fields which uses a Patch function to create a new item into SharePoint list. However for a user it is always creating 4 entries.  It is also throwing an error notification "An error occurred on the server. Server Response: BusinessImpactAnalysisForm failed: The format of value 'HTTPS/1.1 localhost.localdomain 0A7E660C' is invalid."

(screenshot attached).

I have double checked the user permissions.

For other users it only creates 1 entry as it should do.

Any help is much appreciated.

Can you send me the code?

Thanks

Patch( BusinessImpactAnalysisForm, Defaults(BusinessImpactAnalysisForm), { Title: PatchOrgUnit.Text, DeptLocation:'TextInput Location'.Text, OtherLocation: OtherLocation.Text, TeamProductsServices:PatchProducts.Text, StaffWorkRemotely: PatchRemoteWork.Text, FixedLocationStaff:PatchStaffSpecialistEquip.Text, EndUser: PatchEndUsers.Text, DamageImpact:Concat('Damage Impact'.SelectedItems, Value), DamageLikelihood:Concat('Damage Likelihood'.SelectedItems, Value), DamageBreakdownImpact:Concat('DamageBreakdown Impact'.SelectedItems, Value), DamageBreakdownLikelihood:Concat('DamageBreakdown Likelihood'.SelectedItems, Value) }) ;Navigate('Success screen',ScreenTransition.Fade)

On first Glance try using the "Collect" function. The formating is the same except you wont need to specify your record. Patch is good for updating existing records but collect is better to create new ones.
