Is there a way to track who and when users are using a specific model driven app?

see if this may help as well.
In case the Performance insights or Admin Analytics for Power Apps does not fit and you need more advanced, custom solution, first thing that comes to my mind is Azure Application Insights https://docs.microsoft.com/en-us/azure/azure-monitor/app/data-model
You can try set up Azure Application Insights and hook it to custom JS web resource of Model Driven App to take telemetry on any parts of the specific Model Driven App and define any kind of custom criteria you want.

@poweractivate Thanks for the reply, unfortunately admin analytics doesn't go as deep as when a user opens and or uses a specific model driven app.
Do you think you can help walk me through on how to set up azure insights for my app?
