User can add rows where column values will be filled by lookup from another database & more

Can you please help me how to achieve the following situation:

1. This is a user request app to request a service job for equipment installed in customer site.

2. User will input Job No/ Customer Name/ Job site Name/ Job start Date / Job End Date on the top 1/4 of the screen.

3. User will add list of equipment that needs to be serviced. This will be an editable gallery or table where user will input serial no. and get other fields auto populated by lookup from original install base data (Table 1). The equipment that needs to be serviced could be 1 or at max 300 i.e. user can add 300 rows based on his needs. The Column Titles are Serial No./Tag No./Model/ Size/ Color. 

4. If by any chance the lookup data is incorrect user can edit. If lookup data is not available then user will add manually.

5. Upon click of submit, Add Job details explained in point 2, to a SharePoint list (Order List)

6. Upon click of submit, Add equipment details explained in point 3, to a SharePoint list (Equipment List) with Job No. also added to each rows of equipment.
