Using Contest Registration template - Connection couldn't be added

I keep getting an error when trying to 'use' the available Contest Registration tablet template - the prompt reads

 

'Error: The connection couldn't be added.  Please try again.'

 

I tried adding Sharepoint, Azure AD and Office 365 connections to no avail.  Is there something I am missing here?

I ended up going to the admin portal and adding myself as an 'Environment Admin' and an 'Environment Maker'.  I was able to create a new app based off the template afterwards.

Hi there, quick question using this template:

 

I was not able to use the common data service "user is not authorized to read view rtscontact. request denied", so I heve decided to download the contacts list and exported to Excel, then recreate the form suing a new connection with my Onedrive for bussiness, I have to change all of the fields from lowercase to upper case, because the excel file downloaded the template in this way. but I noticed that it has a collection and at this point I am not able to create a record with the form.

 

Could you please point me in the right direction to get this form working?
