I have a list in a SharePoint site and the list form UI is customized using PowerApps.
But if I go to the PowerApps home page (ie. https://make.powerapps.com/) and click Apps on the left menu, I cannot find my app in the list.
In fact, I can only access the app through the SharePoint list, as below

In fact, your custom form is still in the default environment, but you can not accessing it directly(We can only edit the custom form by select "edit the custom form" the in SharePoint site).
I've made a test for your reference:
In the Power Platform admin center, select the default environment for your organization from the list of environments.
Select Power Apps from the Resources section on the right.
Select the app.
Select Details.
Check the App Type.
