create and collect form data

Hi, i would like to create a form that works like a question paper whereby my users would have to answer questions based on the questions displayed. I would also like the records to be stored onto sharepoint after they click a submit button. is there anyway to go around doing this? Eg using flow or anything?

You could use a SharePoint list to store the users' answers.

 

Create columns for each question. Make the questions in the column names. Also you can create different kinds of column types according to what answers you want users to have. For example, the question is "What is your gender?", then you could create this column with Column name is "What is your gender?", and choose to column type as Choice, and the options are "Female" and "Male".

 

After adding all your questions as column names,  then you could create the app using this SharePoint list and let PowerApps auto-generate the app by click on Create an app > Start with your data > SharePoint.
