not showing all the columns when I click on custamize forms from a list

list has below column. please refer screenshot S1.

But when I click on customize forms from power apps from this list, it is not showing any columns in the power apps. Please refer screenshot S2  (except Title) 

Why it is not showing all coumns in powerapps? what need to be done? Please help. Thank you.

Have you clicked the 'Add Field' button at the top of the screen?

Based on your second screenshot, Click on Add Fields , it will reveal the other fields for you to Add to the form.

But why it is showing all the columns for other lists without clicking on add field? 

ad other one is in the middle (highlighted ) why it is showing only title field? why not other columns? Please refer S3

Yes after clicking on add field it is showing the columns, please refer S3

When you say lists, are you referring to the view that you see in SharePoint? PowerApps forms are something different, so you would have to click the 'add field' button.

If you connect to Gallery, field automatically shows. When you insert a form before connecting the Datasource, that is the process you go through to add Fields

When I say other lists, when I try to click on customize forms in sharepoint for other lists, it is showing all the columns in the power apps, I did not have to click on add field. 

Please clarify my other questions as well.

Instead of insert a form before connecting to s Table
