If the form is on a different screen, replace your Icon OnSelect with this

Navigate(Comment_Screen,None,{varID: ThisItem.ID})

OR OnSelect of the icon

Set(gblID,ThisItem.ID)

Form Item

LookUp(
   SPListName,
   ID = gblID
)

and the Visible of the Form

gblID > 0

AppOnStart

Set(gblID,0)

Hi @damiosiks ,

Firstly OnSelect of the icon

UpdateContext({varID:ThisItem.ID})

Then you just need a Form with the fields shown and the DataSource of the SharePoint List and the Item

LookUp(
   SPListName,
   ID = varID
)

and the Visible of the Form

varID > 0

and to hide the form (which you can also put on Screen OnVisible)

UpdateContext({varID:0})
