How to edit an existing SharePoint item using a form

Here is my scenario:

 

I am making an app for travel approval. The front screen has a button, "New Travel Request". When clicked, a form comes up with a title and description for the request. Once those are filled out,the form is submitted to SharePoint. On the next screen, I have a form that I want to use to edit the previously submitted form. 

 

The way I am trying to design this is that the SharePoint ID for the item will be generated when a new request is created. Then, the ID can be provided to the next screen and used to edit the item from SharePoint. The purpose for the two forms is to be able to save and access the travel request at a later time by editing the SharePoint item using a form and the ID. 

 

Will this be possible? Can I provide an ID to a form and then edit the fields that correspond to that ID? Any input/advice?

I noticed the "Item" property on the form. Would utilizing this be going in the right direction?
