A SharePoint custom form Power App for collecting feedback on existing products & processes or for collecting new ideas.
Includes an auto-suggestion feature as users type in feedback or idea titles so if their title looks similar to previous entries, then it provides those previous entries as click-able options, similar the auto-suggestions on the Power Platform Community pages.
Also includes a Vote feature so users can upvote feedback & ideas to give them higher priority, similar to the upvoting on Power Platform Community pages.
Import & Set-Up Information
These are all the columns & column data-types for the list:
IdeaOrFeedback choices are "New_Idea" & "Feedback". SustainOrImprove choices are "Sustain" & "Improve".
I also highly recommend turning on the rich-text feature for the Description column so users can do more text formatting & include links to examples or other supporting sites.

Once you get the list set up and the custom form app in your environment, make sure to change the list name to IdeasAndFeedback. This will allow us to remove the old list reference & create a new list reference for the the app datasource with matching datasource names. That way we don't have to go through each piece of the app to manually replace the list datasource references multiple times.
So go to View > Data sources and remove the old IdeasAndFeedback SP List datasource, then add a new datasource that connects to your new list with the same IdeasAndFeedback datasource name.
That should set everything up for the custom form & list.
Now if you are sharing this with other people in your organization to try to collect new responses, I suggest using the NewForm link.
So go to the default view of the list, then find the part of the list url that reads "/AllItems.aspx" and replace it with "/NewForm.aspx".
That should bring you to a simpler form page that looks like this:
That NewForm url can then be shared by email or any other means with anyone who has access to the list. You may need to make sure you add list permissions like Contribute access for anyone you want to get responses from.
I also like to use the Website option in a new Teams tab to display the form directly in a Teams channel:
