Approval Email error the action could not be completed

When I try and action my approval emails from outlook, the system spends several minutes thinking before a small error message saying "The action could not be completed" appears. My connections all say connected. 

Is anyone else experiencing this issue? Any suggestions for what can be done to resolve it?

My latest test results show that the approval request has been completed in the Approval email.
