Approval email : "The requested action could not be completed"

I created a basic flow that would send an approval email when a new item is added to my custom list in sharepoint. 

 

But although I tried to create several list I keep getting an error when I press the approve or reject button in the email. 

 

In outlook online i get "The action could not be completed."

 

If I click the link in Outlook 2017 I get 

{ "statusCode": 500, "message": "Internal server error", "activityId": "896cb46c-13ee-43e7-a4e1-fc4bd67aed15" }

 

Does anyone know why this is happening ?

This issue has started happening again for me as of May 31st.

This bug disappeared June 1st.

It seems to only occur on the 31st of the month.

I am seeing this issue with a single user in our environment.  What exactly is the issue?  Looks like the above string states this is a bug that has been fixed.  However, I'm running into the same issue but only with a single user.  Any ideas?
