Email lookup/association from a form submission

Hello,

 

I am trying to create a Form->Flow where individuals within my organization can select a name from a list, and an email will be sent to the selected individual once the form is submitted. We are wanting to use forms to standardize the output of the message as well as have some better reporting functions.

 

My desire is that the form submitter could select from a list (i.e. Bill, Steve, Other) and it would send the message to that individual. I would be fine with Flow either looking up the contact from the company directory, pulling the data from a shared document within our group (i.e. Excel, xml, etc.), or using an "if" command to match names to an email address. I also would like to retain the "Other" option should the submitter need to submit the form to an individual that is not on the list.

 

I am not quite familiar enough with how expressions work to know where to start on this. Any assistance would be greatly appreciated.

Could you please share a bit more about the Form that you mentioned? Is it a Microsoft Form?

Further, could you please show more details about your scenario?
