Anyone help with this failure expression

Hi,

Today and yesterday, I am getting approval fails for flows that worked for two months with the following expression:

 

The request failed. Error code: 'XrmApiRequestFailed'. Error Message: 'Request to XRM API failed with error: 'Message: A record with matching key values already exists.
Code: 0x80040237

 

Is this a microsoft problem that I should wait out?

Hi @Sdjackson 

 

Does this message occur in the "Approve" action or one after that process?

 

There's somewhere a double insert with the same key happening, so if you could share the Flow it would help to try to debug it.

What is the action that fails? The "Start and wait for approval"?

Yes it's the start and wait for approval.  I have two that are failing.  One that's using dynamic content and one that's using an email address.  Both are failing.  All my flows without approvals are working fine.
