Attach multiples files uploaded from Form to an approval process in Power Automate

In my case, some files were uploaded from Form and stored in OneDrive, I would like to attached these files for decision email.  I used  "Initialize variable" to store the files, get the files from "Append to array variable" and then attached them in the approval.  But, error message was found, can anyone share your knowledge and experience?

I want to do attached the following files in one go in the approval email:

1. a designated word file (converted from microsoft form for the form content)

2. files uploaded in the form (up to 5 files)

I can just attach either one, but can't both in one go.  

Appreciate if you can share your experience.
