Duplicate Approvals

I've come across this issue in this forum a couple of times but hasn't quite resolved my problem.

I current flow that was built about a year ago is suddenly sending duplicate approval requests to managers via email and teams 'approvals'.

I've stripped down the flow and rebuilt but it hasn't resolved the issue.

I feel like something has changed in the 'start approval' action where the duplicate is originating from. Should I be using something else or is it further down in the 'condition' step?

I used the suggested template from the pre-built library and only added a 'get manager' step, everything else is normal for the template.
