File not found when adding attachment to SPO list from Form

Hi,

I followed this video by @RezaDorrani @(232) How to save Microsoft Forms Responses & Attachments to SharePoint Lists or Libraries & Send Email - YouTube

I need to put the attachments into an SPO list.

Flow works fine for images and pdfs, but when i use a word document it fails with 404 File not found.

If i copy the file path from the failed flow, it opens as expected.

Further down in the comments someone else had the same issue and the suggestion was to use the expression concat('YoursiteURL/Shared%20Documents/Apps/Microsoft%20Forms/Training%20Survey/Question/',item()?['name']) for file path.

I changed mine to this 

concat('MyrsiteURL/Shared%20Documents/Apps/Microsoft%20Forms/Work%20From%20Home%20%28WFH%29%20request/Question/',item()?['name'])

I did this and it does the same thing?

Can anyone please help me fix this?
