Flow created results to duplicate items in a sharepoint list

Hello. I have a simple powerapp that submits entries to sharepoint list test 1. I want whatever is created to get replicated to sharepoint list test 2. When I created a flow, it creates 2 entries to sharepoint list test 2. Not sure where I am doing it wrong.

Here is Sharepoint list test 1: so i just entered 2 entries in powerapps

this is my flow to replicate this to another sharepoint list:

unfortunately, it gets duplicated in sharepointlist test 2. Not sure why

In the flow, you have trigger "When a new item is created or modified". So, you will get all the details of the new item created in Test 1 list.

 

You do not need Get Items action and Apply to each loop.  Remove both. 

 

Just place Create Item action and map the columns with values from the trigger.

 

Because you are using Get Items, it is getting all the items in the Test 1 list and creating in Test 2 for each new item in Test 1 list.
