Forms Flow with Conditional Logic

Hello, 

I'm having trouble creating a stream with Microsoft Forms. I want to write Forms responses in the Sharepoint List, but not all form fields are appearing. What do I do?

Do you see the words "see more" next to the Forms columns grouping title in Dynamic properties?  If so, click on it and you will see additional columns.  If not, please let me know.
