How to disable reason in approvals

Use Case:

all I'm trying to do is have them press a button that is a confirmation that they have done something.

The approval is the closest I've found to this is the approvals but there is the reasoning which I was curious if it could be disabled. and If instead of the button saying submit It said something like "Acknowledged".

@Anonymous  have you looked at the Send email with options action? That should do what you want.
