If Sharepoint List Changes is triggering when nothing changes

I have my Sharepoint list with columns including one "step" which denotes which step a customer feedback is at for responding to.

Initially on creation this is set to "Responsibility to be Assigned" and I have a flow to monitor for "If Sharepoint List changes" with a Condition for "Step Value" contains "Responsibility to be Assigned" With Yes to continue and No to terminate.

In Yes it should then update the "Step Value" to "Responsibility Assigned" > Get Item based on "ID" column > Send an email.

Problem is now (seemingly was OK yesterday) when I create an item or change existing item back to "Responsibility to be Assigned" and have test on it triggers and changes Step Value to "Responsibility Assigned". Even if I turn off the Flow completely, change the fields back and then turn back on when I hit test it changes all items to "Responsibility Assigned" despite nothing having been changed in the list while test is running.

Scratching my head a bit - nothing in list is changing so why is it triggering? Even new events I can't do now as soon as created before assigned they are changing their Step Value as well again when nothing has been changed.

Hopefully some screenshots or more info help:

2. Without doing anything after a couple of minutes it changes to Responsibility Assigned.

3. I can see Flow ran but in the "log" of the event it seems to only show information on the Failure to send email (which is expected as Assigned hasn't been set so there is no email address to input in the To field.

4. How can I see WHY it triggered? From my end nothing changed, no field changed in the list. An item was Created and that triggered it but yesterday that didn't occur. Also as have said even if I turn off the Flow, create the item, turn the flow back on, it still triggers and changes the value before Assigned has been updated.

Also similar is triggers start despite filtering by "Limit Columns by View". Isn't this meant to limit trigger conditions? I only want to monitor id specific fields are updated not the whole item. How is this so hard?

There is literally on the List view in Sharepoint an "automate" or whatever feature that can do exactly this - email a user based on updating a single field. It's there simple as anything all that is lacking is any smarts attached to that to update another field or customise the email sent. I just want that, if a field updates send an email and update another field.

I change Customer Phone Number which is not in that View.

What am I missing? Again. How do I just have a trigger from a single field updating?

Also then below in Update Item I have limited by Sharepoint List View, but then fields that ARE NOT in that View show as trying to update and I am paranoid these are going to overwrite:
