MS Flow Approvals don't work in 20-30% of cases in Canadian Tenants

Today we have noticed that approval tasks never get created in many cases in Canadian non-default Flow Envrionemnts. In about 20-30% of cases approval tasks simply don't get assigned to a user. In these cases we don't get any emails, "Pending Approvals" tab is empty. And the Flow is stuck at the Running state.

 

It used to work fine, but about 4-5 days it became very unstable. We noticed only today, but the history shows that it's been an issue since at least Friday. 

 

We can easily replicate this issue with a really simple Flow wtih a single Approval action. We can replicate it in two totally different Office 365 non-default Canadian envrionments. 

US envrionments work fine.

 

Instance version: 9.1.0.28

We can replicate the issue by using a single account (see screenshot below) We can keep triggering the same Flow and in many cases approval tasks simply will never be created. When it works - we get a task and an email within 1-3 seconds. When it does not work - we can wait for hours and nothing will arrive. Please, note, that we don't modify Flow. We keep running the same one over and over again and get different results. Refer to the screenshots to see how simple our sample is.

We can now say the isue has been resolved without us doing anything. It's been over a week since we last seen missing or super-late approval tasks. 

 

We are also going to close a ticket with MS support.

 

It's still a shame this was happening and noone could explain why.

Have you checked if the issue still exists?

Could please take a try to create a new flow to see if the issue still exists.

This is still an issue. A serious one. It was working fine two days ago. I've tried running the same flow 20 times in a row in two different Office 365 tenants and everything was arriving within 1-3 seconds.

 

Yesterday the issue came back. The approval tasks arrive with a monstrous delay (4-20 hours late) or never arrive at all.

 

The workflow is stuck pretending to be busy at assigning an approval task to a single user. At the same time nothing arrives at all. This is very unreliable, because in more than 50% of cases the tasks gets assigned immedeately. And I am talking about a completely blank workflow with a single action that we run over and over again.
