Missing top of Append Variable in PDF creation.

I have this flow

Work well except for the append var, while it is using HTML to fill in all of the var, it is missing the header pieces of the form

          <p>Safety leave recognition may be given in circumstances that are approved by SST and the employee’s Senior Manager. Awards cannot exceed 24 hours and hours awarded will be commensurate with the effort and outcomes of the event being awarded.</p>

I think I have an issue with the HTML listed above because the first part of the form is not showing as it should.

 

It should look like this

But it is only bringing in the bottom.

Any help would be appreciated.  I am so close to having this all working finally.
