Power Automate approval notification issue

I have a very simple flow.  A SharePoint item is created and an approval is sent out to a single person.  That person gets the approval request in a minute or so after the flow is kicked off.  No issues.  However, for some reason, if they don't approve right away, they are getting two additional notifications at 5:25 p.m. and 5:40 p.m. eastern time.  The approvers will get these notifications at these times every day, until they approve.  The approver can reply to any of the 3 notifications and then they will no longer get notified.  The flow fires only once...I have verified this.  

This just started happening.  Its not only happening with this flow, but all our other flows as well....two notifications sent out at 5:25 p.m. and 5:40 p.m.

What could cause this?

I believe something is going wrong in the internal Power Automate engine.

 

This flow is as simple as it gets.  It fires only once.  The approver gets the initial approval e-mail notification. Then, the approver is re-notified at 5:25 p.m. and 5:49 p.m. every day until they approve.  It happens not only with this flow, but with all flows now.  This never happened before.

In the absence of a recurrence step, and the inability to see whether something else in your environment is causing it, I'd say Microsoft has a bug.

Delete the start and wait for an approval step and substitute the create an approval step and then the wait for an approval step and see if they also generate the same extra notifications.

UPDATE: This was also reported in 2020, and supposedly fixed at that time. Looks like it's back. Solved: Approvals connector in a flow sending same approva... - Power Platform Community (microsoft....

I appreciate the feedback, but I don't want to have to change a ton of approval flows on my end, just to fix a bug on the MS end.  This should work.  Only one notification should go out.  There is nothing else in my flows that would send out these notifications at exactly those times every day again.  I have a call with MS today about this issue.

Any update for this topic? I'm facing the same issue as you described.

This issue was resolved for us in early December when the fix was rolled out.

I’m afraid my Approval workflow triggered two approval request mail to an approver.

Do you have any idea how to fix the problem?

I'm not an expert by any means, but in my experience it often happens when an action is in an "Apply to Each" loop. It's hard to know more without seeing your workflow.

Thank you for the suggestion. Here's attached my flow. I'm afraid I did not "apply to each" in Myflow.
Do you have any idea to avoid getting two (duplicate) approval requests?

Please show what's inside the Supervisor Approval step.

Also, please show the steps of the Flow from one of the test runs where the abnormal output happened. 

This will give a better idea of what's happening.

Is the Mail in the Supervisor Approval step assigned to a group address? The approval would be sent to all the addresses.

Are the two approvals coming at the same time or does one come after the other?

Can you show the outputs of the Supervisor Approval step? (Black out any confidential information)

Hi im having the exact same issue. Multiple notifications on MS teams and email sent to the manager for approvals.
