Power Automate run twice

Good day. I need help with my flow.

1. A MS Form get completed.
2. Then a SP list item get created.
3. User must add approver name to the list on SP.
   1.  In the flow the condition is "IF Approver column change" to start process.
4. Approval process run. (BUT this happen twice)

@Herculas Check your Apply to each loop. What is return type of Refund reason?

Thanks for the reply.
I'm still very new in this.
It is a choice field in the list and in the approval I need to tell them what the reason is, if I choose the dynamic content it adds it to the Apply to each.
