I realised it was because the Flow account didn't have proper write permissions to the list.
Once I gave it Full Control permissions it sent off the flow emails instantly.
Depending on the Flow license involved it can take up to 5 minutes for some actions to happen. That is the standard polling cycle for Flow when using a non-premium license. This is pretty typical. P1 and P2 licenses have a much shorter polling time. So if your regular license is a P2 then mail will be sent within a minute, while a regular user it may take up to 5.
