duplicate approvals when approving using ms teams

Hi everyone,

I built a flow that uses MS Forms and creates a list in SharePoint and sends approval emails to various people and updates the status of the approvals in the SP list. 

I'm noticing that some users are not receiving approval emails or they approve using MS teams and it is not updating the SP list. Are there issues with this setup?
