I'm also getting exactly the same issue on the "Start and wait for an approval" action - with no changes on my end to the config.
This was working fine last week and suddenly stopped just prior to / just after this past weekend.

I can confirm this behaviour as well. Hopefully it's just a bug caused by some change in outlook online.
As I mentioned I had the same issue, but I ran a test tonight, and it seems like it´s working just fine!
Does it also work for the others that had the same issue?
Thank you for the notification. I've tested my problem workflows and can confirm that they now behave as expected. Hopefully this is a fixed issue and won't recur. It would be nice to know what actually happened.
