Today I noticed a change in the approvals workflow behaviour. It seems that the response comments are not being returned correctly. I've included a picture below. This is a field reference rather than the actual content.
I've tested several flows that use this approval workflow, with the same result. I haven't changed anything recently.

I am afraid I can't reproduce a similar issue on my side.
Could you share the specific details of the Flow configuration?
Or do you have any special operations when editing the comment?
Please provide more details so that we can analyze this issue.

Okay, rather than clip all of the flow setup (which contains sensetive information), I've made a copy and stripped everything down to the basics...
The flow creates an approval which has three reply options (Opt in, Opt out, Redirect). I've used the Create an Approval with custom options.
The response processing part is a simple condition cascade...
I've opened the redirect response so that you can see how the response comments are drawn from the dynamic content of the approval (above left).
When I select a file and run the flow the approval request comes to me because I set myself up as the approver in this case...
My response to the request was to opt in. I added test comments in the submit dialogue...
The approval was processed and I was left with this receipt...
You can see that the comments have not been brought forward...
Also, When I process the response...
The comments show up as a field reference rather than content.
This is now affecting all of my approvals flows.
It may help to know that as a large organisation our office 365 governance is operated by a different department. It's quite possible that background changes have been made to my 365 setup that I'm not aware of.
I'm investigation this and will inform if I learn anything. Even if I can cure it at this end and leave pointers for others to follow it'll be something.
I'm still no wiser about the tchnicalities but the issue is related to outlook online.
I tried sending th same approvals to a colleague and it worked perfectly. My colleague uses Outlook 2016. Although the markdown doesn't render very well in the Outlook application, (the original message section renders perfectly), the responder's comments are transported as expected, but here's another thing...
If I open the approval response email using Outlook 2016 the comments come through. If I then open the same response email using Outlook online the comments also come through.
If I open the approval firstly with Outlook online the comments are lost and if I then open the same response email with Outlook 2016 the comments are still lost.
The outcome seems to depend on which application I use to view the email, and in what order?
This has to be an Office 365 permissions issue perhaps?

This is indeed an issue, the Outlook team is working on this. It is a regression on their side, and they are looking into a fix.

Update. I tested Approval Comments and it seems that this issue has been solved. This is my test result:
