Docusign Authentication Failing

See Screenshots Below.

 

This is the "Get Envelope Documents" action. Credentials are confirmed correct.  I've tried going into Manage Connections and clicking "Fix Connection", and deleting/readding the connection. All routes lead to the error in the screenshot.  It sits in the "Updating..." when clicking the "Invalid Connection" link below the account name.

When attempting to delete, re-add the connection, I get the following error:

This might be relevant, but when the above fails, a request named 'login_information' comes back with a 500 and the following response body:
