Problems installing Inspection App



I am the admin for our Teams and am trying to install the Inspections App for us to use, but am getting an error message when I add it in that flashes up briefly and disappears. It looks like this: Screenshot (5).png

I can process the next steps but then this screen comes up with another error message

I can close the error message and carry on working, but when I do, nothing saves and when I click out of that tab and go back in, I have to start from the beginning again. 

I have spent hours trying it in different ways - removing and reinstalling, checking I have full admin rights, on the desktop app, online version, different web browser, setting up a new Team etc, reading forum posts, but nothing is helping. Any solutions? Thanks so much!!
