Request Sign-Off - Approval Email bug

Our team is looking to use the Request Sign-off Flow in SharePoint on shared documents. As a part of our controls, we'll need to enter text in our approval submission in the option text box. When using the automated approval email form to approve the file, we noticed in testing the email would "refresh." Each time it refreshes, the email resets and deletes any text in the option text field that was there prior to the refresh. 

 

Can this be updated to not delete the text? We did not have this experience in the Flow application/website, just the automated approval email.

I have tested on my side that starts the Request sign-off for a file, then approves it in the Email, but not find the text in the option would be delete.Annotation 2019-09-19 130646.pngAnnotation 2019-09-19 130518.pngCould you please share some screenshots about your issue?
