Request Sign-Off - multiples appearing (default Flow)

As another post in the General Flow Discussion mentions, in SharePoint Online in a document library, we are seeing multiples of the default Request Sign-Off appear when you go to an file and select Flow from the item drop-down menu. When I open most of them, I see the "this flow will connect to:" with the Continue button, but one of them takes me directly to the form where I select the Approvers, and start the Flow.


Looks to me like a new one is being created for each person who activates the Request Sign-Off, and then they all appear for everyone. If this is the case, it should hide the ones created by other users and only display the one for the current user. 


To repro:

  1. Create a document library.
  2. Have a few different users create documents and have them each run the out-of-the-box Request Sign-Off Flow.
  3. Go back to this document library, choose a file and you'll see many instances of the Request Sign-off Flow.
