After much trouble shooting i figured out what was cause my new app with its very first form to crash when selecting a data source.
Adding a form and selecting the table/Entity as my datasource PowerApps would crash with the following error.
Here is the table and columns in the table.
It was not until I removed the "Date Only" Data Type did the failure would go away and connect to my Form.
Another observation is the "Status" column name came in with the unique column name instead of display name.
