Unable to create CAre Team for a user in Care Management

Hi,

I came across an issue while creating a Care Team for a user in Care Management portal . 

I have checked the Tables in Power Apps. Everything is filled in. All mandatory fields are filled in. Still I get this error. Kindly help me in getting this issue resolved
