Selective Navigation

Hello,

 

Really hoping someone is able to help. I've got a Powerapp (Custom Sharepoint form) that appears to be working as expected until I go to 'edit' the item.

 

When I create a new item, I'm lead to a kind of 'Homescreen' with buttons navigating to certain screens. This all appears to be working correctly. When I go to 'Edit' an item however, it'll take me to that same homescreen and when I click on one of the navigation buttons it takes me to a blank (Empty) version of the form. This means I am unable to edit an item as it's only showing me the 'new' form. This appears to be the case for each of the forms (And some of them share the same List Items).

 

Is there any way to click to 'Edit' an item in Sharepoint whereby it will auto navigate to the item that has set criteria within a certain field? Or is there any way to simply ensure that if 'edit' item is selected, I can click on the apprioriate button and the pre-entered elements are already in there? I've added some screenshots in the hope that will help and I've put them in sequential order.
