The app didn't start correctly. Try refreshing your browser - Custom Form

We have been using the same Sharepoint List and custom PowerApps form for months.  One user suddenly cannot open the form to complete a request.  However, all other users can open the form with no issues at all.  The error he gets is "The app didn't start correctly. Try refreshing your browser."  We have tried multiple browsers with the same result.  


After searching through the forum subjects, I see that this has happened a lot, but no real resolution or fix for the issue.  Any ideas??

Have you tried clearing the cache and cookies in the browsers?

Yes, we have tried multiple browsers and have tried to clear cookies/cache.  We have also tried resetting Firefox back to factory settings, no luck there either.  No other users are having issues.
