I have an PowerApps Portal with some forms a month ago. Everything works fine.
Today when I created new PowerApps Portal and try to use the existing Form (updated to new Portal) and a fresh new form to insert data to the entity.
But both forms do not show at all in the Portal (only see blank page).
If I setup this form for "Create new record" in the entity list, I an see this form when try to create record for entity list.
Is there something wrong with PowerApps Portal's Form recently? or is there anything that I can try to make this work?

Actually that's the snapshot for the main / root web page, the Localized Content is located on the subgrid:
can you check the configuration on that page
just so you know, whenever we create a Web Page, Power Apps Portals automatically creates a 2nd web page (content), and this is the record who really dictates any content / JS / Forms / CSS / etc of the page.. the parent one is more to hold a few structural elements and partial URL
this is useful when you have a multi-lingual Portals, and then you can define a Web page for each language

Could you please try to upgrade the portal. You might have seen the message on the title bar stating that "New version is available". We had the same issue on production after upgrading everything is back to normal.

I upgraded the portal but I still see the same issue. Can you please let me know if I miss any here?

I tried the same steps here and it worked fine, so it's (probably) not an issue with the Portals Studio.
to better help you, I will ask you to go a bit old school now.. instead of going to the Portals Studio, can you navigate to make.powerapps.com --> apps --> Portal Management
this will open a Model-Driven App / D365 style app with the Portals configuration

Overall here I think the issue might be to your page configuration or which Page Template it's referring to.. depending on what's there I will probably ask you to set the "Full Page" as Page Template

I tried to change to FullPage but I still see the same issue.

Can you navigate to the Localized Content Web page and make sure you have the Basic Form selected there:
have you also cleared the cache? you can do that via Portals Studio --> Sync Configuration

As Oliver mentioned, it works fine for me too. Please select the basic form and clear the cache by going into the following URL
Going to your portal service page: https://<your_portal>.powerappsportals.com/_services/about
Note: you should have environment admin or above privileges to perform the clear cache operation.
