Comments Improvements

1. Allow comments to only be turned on for certain responses (only for rejections)

2. When comments are turned on, update verbiage on task to "Mandatory" from optional

3. When comments are turned on, do not allow task response to be submitted without comments (currently, tasks can be completed without comments but if a comment is not entered the flow fails)
