Power Apps custom connector OAuth 2.0 refresh token

I have implemented a custom connector for power apps with OAuth 2.0 authentication using the generic OAuth 2.0 provider, but the connection fails after the OAuth bearer token expires after 1 hour and the power app does not refresh the token before that.
I suspect that the token refresh url is incorrect.

Can you provide directions of how to construct the refresh url?
I can't find any official documentation regarding this and Active Directory provider in OAuth authentication in Power apps custom connector does not work, which is why I am using the generic OAuth provider.

For reference the connector looks something like the attached image.

I have recently read that the microsoft azure active directory graph api offline_access scope is required for the application (azure enterprise application) to get access to refresh the access by getting a refresh token in the response when authorizing with azure active directory. Is this correct and do I need to setup anything else in order for this to work?

Instead of using below url to get token 

 

https://login.microsoftonline.com/TENENT-ID/oauth2/v2.0/token

 

try this, this will return you the refresh token as well

 

https://login.microsoftonline.com/TENENT-ID/oauth2/token
